The consent form linked below is designed to ensure that families, young people, and others we work with understand their rights and what they are releasing the Federation to do when they sign a media consent form.
Please use this form if you are requesting permission to use someone’s story, image, or creative works to promote a program or the Federation.
To download these logos, use your right mouse button and select “Save Image As” and save a copy to your files. Then, insert it into your document as needed. You can also select “Copy Image” and may be able to paste the image into a document or Canva file.
You may also use the horizontal version of the logo.
